There's no Request Form for this. The roommate must apply online. Go to the Vacancies page under the Annual Rentals menu on our website.

The prospective roommate must consent to, and pay for, the background check.

Have them write in the comments that they are applying to be added as a roommate on your property.

Please refrain from submitting multiple requests, as your request will be sent to the back of the queue.
